Dear Parents of 7th and 12th grade students:
At the end of last school year, we sent out letters informing parents of the required immunizations for students entering the 7th and 12th grades. Our records show that many parents have not sent a copy of their son’s/daughter’s shot record to the school. We understand these are unprecedented times but ask that you make this a priority.
As mandated by the Director of the Ohio Department of Health, students entering the 7th grade are required to have received one dose of both a Tdap and Meningitis vaccine. Students entering the 12th grade are required to have one or two doses of Meningitis vaccine.
Immunizations may be obtained through an appointment at the Putnam County Health Department, Okuley’s Pharmacy, Walmart, a local doctor’s office or other location. Once the immunization has been obtained, please send a copy of the shot record to the High School Office.
According to state law, students who are attending school but are not up-to-date with their immunizations, have a grace period of 14 days to receive them or they are in violation of Ohio law. This means any students without proof of immunizations being on file on September 15, 2020, will be excluded from attending school until receiving them.
