Dear Parents:
It is now time to register your son or daughter for the Fall 2018 Putnam County West soccer season at Continental. Groups are as follows: Grades K-1-2, 3-4, 5-6, 7-8.
The attached form needs to be completed in full (both front and back) with parent/guardian signature. The fee will be $45 per player. The fee provides your child with a new, good quality uniform (which includes a jersey, shorts and socks) and helps cover the cost of team registration, equipment, liability insurance and officials. The registration fee and completed forms can be mailed to the Continental Soccer Club, at the address below, anytime before Saturday, May 12, 2018. Please do not send the form to school with your child. A late fee of $10.00 per child will be assessed if the registration is received after May 12. ABSOLUTELY NO REGISTRATION FORMS ACCEPTED AFTER MAY 31ST. THIS YEAR NO FORMS WILL BE ACCEPTED WITHOUT A PARENTS AND PLAYER SIGNATURE FOR THE LINDSEY’S LAW VIDEO. NO EXCEPTIONS THIS IS REQUIRED BY THE STATE OF OHIO!
Anyone willing to volunteer as a coach, assistant coach or referee, please indicate so on the registration form or contact Josh Crossgrove at 419-796-9166 prior to the May 12 deadline. Please remember your assistance and involvement is needed for the continued success of the Continental Youth Soccer Program.
Practices can not start until August 1st and games will start approximately the middle of August.
