As mandated by the Director of the Ohio Department of Health, students entering the 7th grade are to have received a Tdap or Td booster, with the Tdap booster being preferred because it also provides protection against pertussis or whooping cough. Students entering the 12th grade are required to have one or two doses of Meningitis vaccine. It protects against four strains of Meningitis.**
**Immunizations may be obtained through an appointment at the Putnam County Health Department during their immunization clinics, Okuley’s Pharmacy or local doctor offices. Once the immunization has been obtained, please send a copy of the shot record to the High School Office.**
