Vaccination Requirements for the 2024-2025 School Year

All incoming 12th graders for the 2024-2025 school year are required to have 2 doses of meningitis vaccine for entry into 12th Grade. There are other vaccinations that are equally important and recommended. Please contact the health department if you have any questions at 419-523-5608.
All incoming 7th graders for the 2024-2025 school year are required to have one dose of both a Tdap and Meningitis (MCV4) vaccine for entry into the 7th grade. There are other vaccinations that are equally important and recommended. Please contact the health department if you have any questions at 419-523-5608.
