Family Care Office In Continental To Close

Patients of Continental’s Putnam County Family Care office recently received a letter announcing that “after careful evaluation, Lima Memorial Physicians has decided to close the Putnam County Family Care office in Continental.” Patient records will remain with the Lima Memorial Physicians and patient care will be transitioned to the Putnam County Family Care Ottawa location at 102 Putnam Parkway in July (ph 419-523-9632). Dr. Jennifer Maag will continue to care for patients on a full-time basis later in the summer, after a brief family leave, at the Ottawa location.

“It has been a joy and a pleasure to serve the community of Continental for the past few years. I truly hope that you will continue to allow me to be your family physician in the future. I remain committed to providing you and your family with outstanding care. I hope to see you at Putnam County Family Care in Ottawa.” — Dr. Jennifer Maag.

    I’m sorry to see this happen, I really like having Dr Maag here close to home.
    I know of several elderly people who are very sad to see another Dr leave Continental.
    Is there plans for another Dr to come in?
